PolarSat RMA Request Form

To receive an RMA number, simply fill out the form below, print this page, and click on the 'Submit' button. Once the request is received, a customer service representative will contact you and provide an RMA number. Save your printed copy, you will need to include it in your shipment with the RMA numbers filled in. Be sure to include either a phone or fax number, as well as an email or mailing address.
